Please bring the following to your appointment if possible: 

• Your referral/request form

• Copies of all relevant reports

• List of all current medications and dosages

• Medicare, pension card or DVA card

• Any questions you may have

Fees

Payment is expected at the time of the appointment and can be made by Visa, Mastercard, EFTPOS and cash.
